All you have to do is fill out the form below, submit it, and print out the sales sheet that comes up on your screen. Then give the sales sheet to someone at the restaurant. If they give us your information when they sign up, we will pay you $30.00. It really is that simple.

Q:    Who Do I give it to?

        A:    The best option is to give it to the owner or manager so you know that it is being seen by the best person. That isn't

               always possible so feel free to give it to anyone at the establishment. Below are some examples.

                    - Cashier

                    - Server

                    - Delivery Driver

                    - You can even leave it on the table with your tip.

Q:    What if the restaurant doesn't give you the referral sheet I gave to them?

        A:    All of your information is stored in a data base when you fill out the form. We will ask the restaurant how they heard 

                about us and if they say they were given a referral sheet, we will check the data base to see who gave it to them. In

                the event that more than one person referred them we will do a random drawing to determine who gets the referral.
